脑钠肽水平与高血压严重度及左室结构与功能关系的研究

         

摘要

Objective To study the relationship of the level of brain natriuretic peptide (BNP) with the severity of essential hypertension and left ventricular structure and function.Methods Thirty-one healthy persons (contol group) and 86 hypertensive patients were enrolled and divided into different subgroups according to the degree of hypertension and the risks of cardiovascular disease.The levels of BNP,left ventricular mass index (LVMI),interventricular septal thickness (IVST),left ventricular posterior wall thickness (LVPWT),left ventricular diastolic diameter (LVDD),left ventricular ejection fraction (LVEF) and E/A of each group were detected and analyzed.Results The level of BNP in hypertensive patients was significantly higher than that in control group,and it was increased with the degree of risk(P<0.05 or P<0.01).The level of BNP was positively associated with LVMI,IVST,LVPWT,LVEDD and negatively associated with E/A (all P< 0.05,r=0.485,0.511,0.507,0.463,-0.470 respectively).The level of BNP in the patients with grade 2-3 hypertension with left ventricular hypertrophy (LVH) was significantly higher than that in the hypertensive patients without LVH(P<0.01).Conclusions The level of BNP is a biochemical marker that can be used to estimate the degree of hypertension and evaluate the left ventricular structure and function.%目的 探讨脑钠肽(BNP)水平与高血压严重程度及左室结构与功能的关系. 方法 选取31例健康体检者(对照组)和86例原发性高血压患者,进行高血压分级和心血管疾病危险度分层.检测并比较各组BNP水平以及左室心肌质量指数(LVMI)、室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、左室舒张末期内径(LVDD)、二尖瓣舒张早期血流峰值(E)/二尖瓣舒张晚期血流峰值(A)(E/A)、左室射血分数(LVEF). 结果 高血压患者BNP水平明显高于对照组,随着危险度增高,BNP水平也不断升高(P<0.05或P<0.01).BNP水平与LVMI、IVST、LVPWT、LVEDD均呈显著性正相关,与E/A呈负相关(P均<0.05).除1级高血压患者外,2级和3级高血压患者中伴左室肥厚者的BNP水平均较无左室肥厚者明显升高,差异有统计学意义(P<0.01). 结论 血浆BNP水平可作为评估高血压患者病情、评价左室结构与功能的一种生物学标志物.
