Objective To compare the application value of stapler with traditional manual suture in radical gastrectomy . Methods 56 cases of gastric cancer patients were divided into stapler anastomat abdominal incision group and traditional tran -sthoracic or abdominal operation group .The operation effects ,complications and follow-up results of the two groups were observed . Results In stapler group,operation time (67.3 ±13.3 ) min,gastrointestinal function recovery time (51.3 ±5.6) h,hospital-ized time (6.8 ±2.3) d,they were all significantly less than those of manual suture group (P<0.05 ).Postoperative complica-tions was also significantly less than the manual suture group (P<0.05 );local metastasis rate,distant metastasis rate,2 years recurrence rate,2 years survival rate of the stapler group and those of the manual suture group had no significant difference .(P>0.05 ).Conclusion Stapler transabdominal radical gastrectomy has shorter operation time ,less bleeding volume,rapid recovery and fewer complications .When conditions permit ,reasonable application is suggested .%目的：比较吻合器与传统手工缝合在胃癌经腹切除术中的应用价值。方法对56例胃癌患者，分别行吻合器经腹切口和传统经胸或胸腹手术，观察2组手术效果及并发症情况。结果吻合器组手术时间为（67．3±13．3） min、胃肠功能恢复时间为（51．3±5．6）h、住院时间为（6．8±2．3）d 、均明显短于手工缝合组（P＜0．05），术中出血量明显少于手工缝合组（P＜0．05）；术后并发症也明显少于手工缝合组（P＜0．05）；局部转移发生率、远处转移发生率、1和2年复发率、1和2年生存率，与手工缝合组比较无显著性差异（P＞0．05）。结论吻合器经腹切除胃癌手术具有手术时间短、出血量少、恢复快、并发症少等优点，在条件许可的情况下，建议合理应用。
