目的：观察连枷胸围术期服用盐酸氨溴索对肺部及呼吸功能保护效果。方法选择2012年1月—2013年12月笔者所在医院收治的接受手术内固定治疗的连枷胸患者72例，随机分成研究组和对照组，每组36例；两组患者在围术期均给予常规治疗，研究组给予盐酸氨溴索（30 mg，静脉滴注，3次／d）干预，比较两组患者咳痰情况的变化、抗菌药物的使用天数以及肺部并发症的发生情况。结果与对照组相比，研究组在术后第5和第7天的咳痰情况均明显改善（P＜0．05和P＜0．01），抗菌药物的使用天数明显减少（P＜0．05），肺部感染和肺不张等并发症的发生率明显降低（P＜0．05）。结论在连枷胸围术期，应用盐酸氨溴索能有效减轻肺损伤程度，改善呼吸系统症状，促进肺功能恢复，并减少术后肺部并发症的发生。%Objective To observe the protective effect of ambroxol hydrochloride on lung and respiratory function at perioperative period of flail chest. Methods Seventy-two patients with flail chest were randomly divided into two groups (n=36,for each). At their perioperative period,the patients of ambroxol group were treated with ambroxol hydrochloride on the basis of the conventional treatment of control group. The changes of sputum and its expectoration,the days of using antibiotics and incidence of pulmonary complications were compared between both groups. Results Compared with control group,the patients treated with ambroxol got a better result. The sputum and its expectoration were improved,the days of using antibiotics was shortened,and incidence of pulmonary complications was reduced. The differences were significant statistically (P<0.05). Conclusion Perio-perative use of ambroxol hydrochlorid for flail chest patients is worthy clinically to improve respiration and reduce pulmonary complications.
