目的:探讨血清N末端B型钠尿肽前体(NT-proBNP)与肌钙蛋白I(cTnI)联合检测对慢性心力衰竭(CHF)患者诊断的临床意义.方法:随机抽取100例经临床确诊为CHF的入院患者作为慢性心衰组,并按照美国纽约心脏病协会(NY HA)的心功能分级方案对患者的心功能进行分级(Ⅱ 、Ⅲ 、Ⅳ),选择同期健康体检人群30例作为对照组;分别采用对抗体夹心免疫法和胶乳增强免疫比浊法,定量检测两组的血清N T-proBN P及cT nI值,并对两组检查结果进行统计学分析.结果:慢性心衰组的血清NT-proBNP及cTnI含量均高于对照组(P<0.05);慢性心衰组各级别患者的血清NT-proBNP与cTnI含量存在统计学差异(P<0.05),且两两之间比较血清cTnI在心功能Ⅱ级和Ⅲ级之间没有统计学差异(P>0.05),其余各组间比较均存在统计学差异(P<0.05);两种指标联合检测的阳性率高于任一单项检测的阳性率(P<0.05).结论:血清NT-proBNP和cTnI联合检测对慢性心衰患者的诊断有重要的临床意义.%Objective:To investigate the clinical significance of B type natriuretic peptide precursor of N type natriuretic peptide (NT-proBNP) combined with troponionI(cTnI) in diagnosis of chronic heart failure(CHF) .Meth-ods :Randomly selected 100 cases of clinically diagnosed CHF patients admitted to hospital as the group of chronic heart failure and in accordance with the New York Heart Association (NYHA) heart function classification scheme on the patient's heart function were (II ,III ,IV) classification ,choosed healthy people over the same period as the control group with 30 cases ;were used on antibody sandwich immunoassay and latex enhanced immune turbidimetry and quantitative detection of two groups of serum NT proBNP and cardiac troponin I value ,and examination of the two groups of the results were statistically analyzed .Results :In patients with chronic heart failure of serum NT proBNP and cardiac troponin I content were higher than control group (P< 0 .05);at all levels of the patients with serum NT proBNP and cardiac troponin I in chronic heart failure group also existd difference (P< 0 .05) .And there was no difference between the two levels of serum cTnI between heart function and grade II (P> 0 .05) ,and there were differences among the other groups (P< 0 .05) .The positive rate of the combined detection of the two indica-tors was higher than the positive rate of either individual detection (P< 0 .05) .Conclusions :A combined detection of serum NT-pro BNP and cTnI is of great clinical significance .
