目的:探讨腮腺浅叶切除术中沿面神经总干顺行解剖面神经方法的手术效果.方法:对我院收治的50例腮腺包块患者行腮腺包块及浅叶切除术,所有患者术中均采用经面神经总干顺行解剖的方法解剖面神经,游离腮腺浅叶并切除,分析手术效果.结果:50例患者术中均经主干游离面神经, 3例患者术后出现轻度面瘫症状,3月内均完全恢复,1例患者出现腮腺瘘,3周后恢复,无耳垂麻木、面部畸形等并发症,无肿瘤复发病例.结论:沿面神经总干顺行解剖面神经分支的方法解剖标志清晰、解剖路径顺畅、切除腮腺包块彻底,是一种较好的减少面神经损伤的手术方式.%Objective:To discuss the shallow lobe parotidectomy operation effect with the posive way facial nerve dissection method alongside the general stem.Method:To hold the parotid gland lump and shallow lobe parotidectomy for the 50 cases of the parotid gland lump ,all the patients underwent the posive way facial nerve dissection method alongside the general stem to separated the parotid shallow lobe and analysed the operation effect.Results: 50 patients facial nerve all be successfully separated through the positive way method,3 cases occurred slight facial paralysis symptom and recovered completely with 3 months,1 patient occurred parotid gland fistula sumptom and recovered with 3 weeks,without the ear lobe numbness、facial malformation complication occurred,without recurred cased during the follow-up period.Conclusion: The positive way facial nerve dissection method alongside the general stem in the shallow lobe parotidectomy show many advantages such as clear anatomical token、fluent anatomical tract、completely resection about the parotid gland lump,the method is a kind of good way that reduce the facial nerve injury incidence.
