Objective :To evaluate the clinical effect in treating early diabetic nephropathy in elderly com-bined application of ARB and ACEI drugs .Methods :90 patients with early diabetic nephropathy were divided into 3 groups ,were given losartan (group ARB ,30 patients) ,Benazepril treatment (group ACEI ,30 patients) and val-besartan in combination with Benazepril therapy (combined group ,30 patients) ,all the patients treatment clinical were 3 month .The three groups were compared before and after treatment of systolic blood pressure (SBP) ,dias-tolic blood pressure (DBP) ,creatinine (CR) ,blood urea nitrogen (BUN) and urinary protein and improve treatment during the incidence of adverse drug reactions .Results :after 3 months of treatment of three groups of patients after SBP ,DBP and urinary protein were significantly improved compared with before treatment ,CR ,BUN compared with those before treatment had no obvious improvement ,but before treatment and after treatment three groups of SBP ,DBP ,CR ,BUN showed no statistical difference ,only proteinuria group than in the other two groups signifi-cantly improve .At the same time ,adverse reactions during treatment of patients in the three groups were no signifi-cant difference between the drugs .Conclusion :early application of ARB in patients with diabetic nephropathy and ACEI drug treatment can significantly improve the clinical treatment effect of patients ,and does not increase the in-cidence of adverse drug reactions .%目的：分析联合应用 ARB及ACEI药物治疗老年早期糖尿病肾病的临床疗效。方法：随机将90例老年早期糖尿病肾病患者分为三组，分别给予缬沙坦（A RB组，30例）、贝那普利（ACEI组，30例）及缬沙坦联合贝那普利（联合组，30例），所有患者治疗3个月。对比三组患者治疗前后收缩压（SBP）、舒张压（DBP）、肌酐（CR）、尿素氮（BU N ）及尿蛋白改善情况及治疗期间药物不良反应发生情况。结果：三组患者经3个月治疗后SBP、DBP及尿蛋白较治疗前明显改善，CR、BUN较治疗前无明显改善，但治疗前及治疗后三组SBP、DBP、CR、BUN比较均未见统计学差异，仅尿蛋白联合组较其余两组明显改善。同时治疗期间三组患者药物不良反应比较未见统计学差异。结论：早期糖尿病肾病患者联合应用ARB及ACEI药物治疗可显著提高患者的临床治疗效果，且并不提高药物不良反应发生率。
