Objective: To analyze the intervention effect of the information-based self-service health detection mode in the community diabetic management to provide a basis for the promotion of the new community diabetic management mode.Methods: One thousand and iffty-ifve diabetic patients in the Longbai Community ifle management in Minhang District of Shanghai were selected and 570 cases were as an intervention group, who were managed with the information-based self-service health detection mode under the guidance of the general practitioner assistants since Jan 2012 and 485 cases were as a control group and received the routine health management service. After 12 months intervention, the diabetic self-management behavior score and the changed degree in the laboratory indicators of two groups were compared to the baseline.Results: After12 months intervention, the behavior score of the diabetic self management, fasting blood glucose, diastolic blood pressure, ratio of urinary albumin to creatinine and glycosylated hemoglobin were improved compared with the baseline in two groups and the improved degree in the intervention group was better than in the control group in the above indexes (P<0.05).Conclusion:The information-based self-service health detection mode can promote the self management behavior of the diabetic patients, improve the ability of the self-management, enhance the management quality and save the community health human resources.%目的:分析信息化自助式健康检测模式在社区糖尿病患者管理中的干预效果,为推广新型社区糖尿病管理模式提供依据.方法:选取上海市闵行区龙柏社区建档管理的糖尿病患者1 055例,分为干预组570例,自2012年1月起接受全科医生助理指导下的信息化自助式健康检测模式管理;对照组485例,同期接受常规管理服务.比较干预12个月后两组糖尿病自我管理行为评分和各项实验室指标较基线时的改变程度.结果:干预12个月后,两组在糖尿病自我管理行为评分、空腹血糖、舒张压、尿微量白蛋白与尿肌酐比值以及糖化血红蛋白较基线时均有所改善,干预组上述指标的改善程度均优于对照组(P均<0.05).结论:信息化自助式健康检测模式可促进糖尿病患者改善自我管理行为、增强自我管理能力、提升对糖尿病的管理质量、节约社区的卫生人力资源.
