Objective:To determine the effect of the model of integration and arrangement of the prescribed drugs at patients’ home through evaluation of the satisfaction of the patients and their family members, the rate of drug leakage and other indicators. Methods:The general practitioners visited the patients’ home and integrated and arranged the prescribed drugs for them regularly. After tracing the practice for 1~2 years, the questionnaires survey was conducted and the leakage rate and patients’ satisfaction were analyzed for the feasibility of this service. Results:After carrying out the mode, the drug leakage rate of the patients dropped from 86.7%to 4.0%, and 97.3%of the patients and their families were satisifed with this service. Conclusion:Integration and arrangement of the prescribed drugs for the incapacitating elderly at their home reduces the leakage rate of drugs, which raises their quality of life, improves their awareness of medication, delays complication, and reduces the burden on their families. It’s a good solution to the problem of the elderly medication.%目的：通过评价患者及家属对居家排药服务的满意度和药物漏服率等指标判断排药模式的效果。方法：开展全科医生对患者的用药方案整合及定期上门排药服务，经过1～2年的排药服务跟踪后，进行问卷调查，针对上门排药前后的漏服率和满意度等进行统计，分析此项服务的可行性。结果：医生定期上门排药后，患者的漏服率由86.7%下降为4.0%，97.3%的患者及家属对此项服务表示满意。结论：对失能老人提供上门排药服药能有效的降低药物漏服率，使老年患者的生活质量及按时服药的认知度得到改善，延缓并发症，减轻家属负担，很好的解决了老年人的服药问题。
