Objective To investigate the clinical effect of argon knife combined with high frequency electric knife and cryotherapy in the treatment of endobronchial tuberculosis .Methods A retrospective analysis was con-ducted in 110 patients with endoscopic tuberculosis (October 2016 to May 2017) ,of which 50 patients were trea-ted with traditional 2H RZE/10H R standard chemotherapy regimen (chemotherapy group) ,60 patients on the basis of standard chemotherapy with argon knife combined with high frequency electric knife and cryotherapy ther-apy (observation group) .And the effects of treatment were compared between the two groups .Results The spu-tum negative conversion rate of the observation group was 87% ,which was higher than that of the chemotherapy group(64% ) ( P <0 .05);The disappearance rate of bronchial stenosis in the observation group was 52% ,the disappearance rate of atelectasis was 48% ,and the proportion of lung lesion> 1/2 was 68% ,which was higher than that of chemotherapy group 32% ,26% ,48% ,respectively ( P<0 .05) .After treatment ,the values of FEV1 and FVC in the observation group were significantly higher than those in the chemotherapy group ( P<0 .05). In the observation group ,the effective rate was 42% ,57% effective ,2% effective ,the effective rate was 22% ,68%and 10% in the chemotherapy group .The total effective rate was 98% in the observation group and 90% in the chemotherapy group ,the difference was not statistically significant ( P>0 .05). There was no statistically signifi-cant difference between the 20% of the total adverse events in the observation group and 16% in the chemotherapy group ( P >0 .05) .Conclusion The effect of argon knife combined with high frequency electric knife and cryo-therapy in the treatment of patients with endobronchial tuberculosis was better than that of chemotherapy alone .%目的 探讨氩气刀联合高频电刀及冷冻疗法治疗支气管内膜结核的临床效果.方法 选取我院既往收治的110例支气管内膜结核患者(2016年10月至2017年5月)进行回顾性分析,其中50例患者采用传统单纯2H RZE/10H R标准化疗方案治疗(化疗组)、60例患者在标准化疗基础上采用氩气刀联合高频电刀及冷冻疗法治疗(观察组),对比2组治疗效果.结果 观察组患者的痰菌转阴率87%高于化疗组的64%(P<0.05);观察组患者的支气管狭窄消失率52%、肺不张消失率48%、肺部病灶吸收>1/2所占比例68%均高于化疗组患者的32%、26%、48%,差异均具有统计学意义(P<0.05);治疗后,观察组的第1秒用力呼气容积(FEV1)、用力肺活量(FVC)测定值均显著的高于化疗组(P<0.05);观察组患者的显效57%、有效42%、无效2%,化疗组显效22%、有效68%、无效10%;观察组总有效率98%与化疗组的90%比较,差异无统计学意义(P>0.05);观察组总的不良反应发生率20%与化疗组的16%比较差异无统计学意义(P>0.05).讨论氩气刀联合高频电刀及冷冻疗法治疗支气管内膜结核患者的效果优于单纯化疗,同时具有较高治疗安全性.
