目的：观察中青年高血压合并不稳定型心绞痛患者动态脉压、踝臂指数的变化，并探讨其与血清基质金属蛋白酶‐9（MMP‐9）、超敏C反应蛋白（hs‐CRP）水平的关系。方法选择中青年高血压合并不稳定型心绞痛患者102例作为观察对象，采用酶联免疫吸附法（ELISA）测定MMP‐9与hs‐CRP水平与同期健康体检者94名对照。将中青年高血压合并不稳定型心绞痛患者按动态脉压进行分层，分为4个水平30～40 m m H g、41～60 mmHg、61～80 mmHg、≥81 mmHg ；踝臂指数分为1．00～1．40、0．90～0．99、＜0．903种程度，比较不同动态脉压分层和踝臂指数时MMP‐9、hs‐CRP水平的变化，并进行MMP‐9与hs‐CRP之间的直线相关和回归分析。结果①中青年高血压合并不稳定型心绞痛患者MMP‐9、hs‐CRP水平明显高于对照组；②动态脉压越大，中青年高血压合并不稳定型心绞痛患者血清MMP‐9、hs‐CRP水平越高；③踝臂指数越小，血清MMP‐9、hs‐CRP水平越高；④直线相关和回归分析表明MMP‐9、hs‐CRP呈显著正相关。结论①血清MMP‐9、hs‐CRP水平升高与中青年高血压合并不稳定型心绞痛密切相关；②动态脉压、踝臂指数与MMP‐9、hs‐CRP密切相关。%Objective To observe of ambulatory pulse pressure(APP) and ankle brachial index(ABI) in the middle‐aged patients with hypertension and unstable angina pectoris(UAP) ,and to probe into relationship of am‐bulatory pulse pressure and ankle brachial index with matrix metalloproteinase‐9(MMP‐9) and high‐sensitive C‐re‐active protein(hs‐CRP).Methods One hundred and two patients in the middle‐aged with hypertension and UAP and 94 synchronously examined healthy people (controls) were collected .The levels of MMP‐9 and hs‐CRP were detected by enzyme‐linked immunosorbent assay(ELISA) in all the subjects .The patients with hypertension and UAP were divided into 4 groups according to the level of APP(31‐40 mmHg ,41‐60 mmHg ,61‐80 mmHg and ≥81 mmHg respectively);Based on the level of ABI (1 .00‐1 .40 ,0 .90‐0 .99 ,< 0 .90) ,changes of the level of MMP‐9 and hs‐CRP were compared among the patients with different APP and ABI .Linear correlation and re‐gression analysis were performed for MMP‐9 and hs‐CRP .Results ①The level of MMP‐9 and hs‐CRP were sig‐nificantly higher in the middle‐aged patients with hypertension and UAP group than those in the control group .②MMP‐9 and hs‐CRP increased when APP increased in patients with hypertension and UAP ;③ MMP‐9 and hs‐CRP increased when ABI reduced;④ Linear correlation and regression analysis results showed that the levels of MMP‐9 was significantly positively correlated with the levels of hs‐CRP .Conclusion ①The increase of the level of MMP‐9 and hs‐CRP was closely related with the middle‐aged patients with hypertension and UAP .②APP and ABI may reflect the level of MMP‐9 and hs‐CRP .
