Objective To evaluate brachial‐ankle pulse wave velocity (baPWV ) and ankle brachial index (ABI) values in diagnosis of coronary artery lesions .Methods One hundred and thirty‐two patients ,who under‐went coronary angiography ,ABI and baPWV examination ,were selected from May 2012 to May 2014 in our hos‐pital .According to the results of coronary angiography ,the patients were divided into non CHD group (GroupⅠ) ,coronary heart disease ,single vessel disease group (Group Ⅱ) ,coronary heart disease ,two vessel diseases group (Group Ⅲ) and three vessel diseases group (Group Ⅳ ) .The differences of ABI and baPWV of 4 groups were analyzed .Results There were significant differences between non CHD group ,single vessel disease group , double vessel lesions ,three branch lesions group in baPWV (P<0 .05) .The levels of baPWV were positively cor‐related with Gensini scores in three coronary heart disease groups .However ,significant differences in ABI were observed only in Group Ⅳ ,comparing with other three groups (P<0 .05) .In this group ,ABI can be used for di‐agnoses of three coronary artery branches lesion with sensitivity of 57% and specificity of 63% .Conclusion baP‐WV rise reflects early stage of coronary artery disease ,and was correlated to the severity of coronary artery dis‐ease .ABI decrease indicates more advanced coronary artery lesion ,reflecting the severe lesions of coronary artery .%目的：评价肱‐踝脉搏波传导速度（baPWV ）与踝臂指数（ABI）对冠状动脉病变程度的诊断价值。方法选取我院2012年5月至2014年5月同时接受冠状动脉造影、ABI和baPWV检查的患者132例，根据冠状动脉造影结果，将患者分为非冠心病组（Ⅰ组）、冠心病单支血管病变组（Ⅱ组）、冠心病2支血管病变组（Ⅲ组）及3支病变组（Ⅳ组）。分析4组 ABI和 baPWV的差异。结果 baPWV在非冠心病组、单支病变组、2支病变组、3支病变组组间相比差异均有统计学意义（ P＜0.05），冠心病各组 baPWV 水平与Gensini积分呈正相关。ABI仅在3支病变组与其他3组相比差异有统计学意义（ P＜0.05），本组ABI诊断冠状动脉三支病变的敏感性为57％，特异性为63％。结论 baPWV升高可以反映冠状动脉早期病变，且与冠状动脉病变严重程度呈正相关；ABI降低是冠状动脉病变较晚期的指标，反映冠状动脉严重病变。
