In terms of the source of supply chain risk, the paper claims that the risk of supply chain comes from the external and internal uncertainty of supply chain, and reviews relevant documents. Three risk types are presented in light of the source of the supply chain. Though establishing the assessment model, and using different score to cope with the different risk status membership. By the fuzzy operation, we can get the risk value, so as to offer a new inspiration for enterprises’s risk assessment on supply chain.%本文指出供应链风险主要来源于供应链内外部的不确定性，并回顾了相关文献，将其分为3个风险类型，依此构建评估模型。通过不同风险评分对应不同风险状态的隶属度，运用模糊运算，得出风险水平值，为企业供应链风险评估提供一种新的思考。
