Two principal pathways exist for the histogenesis of vulvar squamous cell carcinomas (SCC), one of them unrelated to human papillomaviruses (HPV). These HPV-negative vulvar SCC are observed predominantly in older women. We observed vulvar carcinomas in the same location below the clitoris and with similar clinical aspect in the younger woman. All SCC were HPV-negative. Delay in diagnosis and therapy was characteristic. The operative procedure and results were similar to the results of patients with SCC not located near the clitoris.
