Objective To evaluate the clinical therapeutic effect of minimally invasive internal fixation assisted by arthroscopy on intra-articular fracture of knee joint. Methods Clinical data of 76 cases of fresh closed intra-articular fracture of knee joint from April,2011 to August,2014 in our hospital were collected for retrospective analysis. Patients received arthroscopy were set as observation group and patients received open surgery were control group. The data of surgery duration,amount of bleeding in surgery,length of stay,bone healing time,HSS and ROM scores at 1 month before and after surgery, Lysholm knee score before surgery and after surgery of internal fixator removal and complications,were recorded in detail and used for comparative analysis. Results The surgery duration,amount of bleeding in surgery and bone healing time of observation group were significantly lower than those of control group (P<0.05). HSS score at 1 month after surgery and Lysholm score of observation group were significantly higher than those of control group (P<0.05). No significant difference in ROM score (P>0.05) but significant differences in good rates of HSS,ROM and Lysholm scores (P<0.05) were observed between these two groups (χ2=8.230,10.565,9.390;P=0.041,0.014,0.025,respectively). The rate of complications after surgery of observation group was significantly lower than that of control group with statistical significance (P<0.05). Conclusions Minimally invasive internal fixation assisted by arthroscopy is suitable for treating intra-articular fracture of knee joint,with the advantages of little trauma,quick healing,less complications and good recovery for quality of life. It has great value in clinical application.%目的 分析关节镜下微创内固定治疗膝关节内骨折的临床疗效及优势.方法 收集2011年4月至2014年8月新鲜闭合性膝关节内骨折患者76例,行关节镜微创内固定术患者38例为观察组,行切开复位内固定术患者38例为对照组.比较两组患者手术时间、术中出血量、住院时间、骨性愈合时间、术前及术后1个月的HSS评分和ROM评分,术前及取除内固定术后的Lysholm膝关节评分及并发症等情况.结果 观察组患者手术时间、术中出血量及骨性愈合时间显著短于或少于对照组(P<0.05),术后1个月HSS评分和Lysholm评分显著高于对照组(P<0.05),ROM评分差异无统计学意义(P>0.05);两组患者HSS评分、ROM评分和Lysholm评分优良率比较,差异有统计学意义(χ2=8.230,10.565,9.390;P=0.041,0.014,0.025);观察组患者术后并发症发生率显著低于对照组,差异有统计学意义(P<0.05).结论 关节镜引导下微创内固定治疗膝关节内骨折具有创伤小,恢复快,并发症少,患者生活功能恢复好等优点,具有较高的临床应用价值.
