西维来司钠对ANP肺损伤保护作用及其机制研究

         

摘要

Objective To study the protective effect of sivelestat against acute necrotizing pancreatitis associated lung injury in SD rats. Methods 45 healthy adult SD rats randomly divided into 3 groups: control group, model group, medication group, each group of 15 only. Retrograde injection of 5% sodium taurocholic acid pancreatic and SAP model was established. Drug groups on the basis of model sivelestat intervention, control group and model group was given amount of saline solution. Detection between groups at 6 h, 12 h, 24 h point measuring serum amylase, arterial blood oxygen partial pressure, lung tissue malondialdehyde (MDA), tumor necrosis factor (TNF alpha), myeloperoxidase (MPO) and neutrophil elastase (NE) content, then observe the pathological changes of pancreas and lung. Results The histologieal injury of pancreas and lungs was milder than ANP group. PaO2 was higher than ANP group, serum amylase were lower than that of ANP group.NE of rat lung tissue, MDA, MPO level as the molding time increases, with the extension of ONO, the treatment group compared with ANP group, the level of MDA, MPO and NE lung tissue is low. Conclusions Treatment with Sivelestat sodium effectively inhibits the release and expression of proinflammatory cytokine in ANP lung injury of rats and relieved lung pathological injuries.%目的:探讨西维来司钠对SD大鼠重症急性胰腺炎肺损伤的保护作用。方法健康成年SD 大鼠45只随机分成3组:对照组、模型组、药物组,每组15只。腹腔注射5%牛磺胆酸钠建立SAP模型。药物组在模型基础上给予西维来司钠干预,对照组和模型组给予等量生理盐水。检测各组在6h、12h、24h时点血清测淀粉酶,动脉血氧分压,肺组织丙二醛(MDA),肿瘤坏死因子(TNF),髓过氧化物酶(MPO)和中性粒细胞弹性蛋白酶(NE)的含量,光镜下观察胰腺和肺的病理变化。结果同期胰腺、肺病理改变ONO治疗组比ANP组明显减轻,且PaO2均高于ANP组,血清淀粉酶均低于ANP组。大鼠肺组织的NE、MDA、MPO水平随着制模时间的延长而升高,ONO治疗组与ANP组相比较,肺组织MDA、MPO和NE水平均较低。结论西维来司钠能明显抑制ANP肺损伤大鼠促炎细胞因子的释放和表达,起到对急性胰腺炎肺损伤进行保护作用。
