Objective ] To study the situation of resistance breath improvement on pulmonary function of elderly patients with abdominal operation before and effect on pulmonary complications of patients after operation . [Method ] 50 elderly patients (age ≥ 65 years ) in surgical department of our hospital from 1-1-2013 to 31-12-2013 who received anesthesia in upper abdominal operation were randomly divided into two groups .The control group was received preoperative routine intervention . The experimental group was treated with routine intervention combining with resistance exercise breath method .Upstairs dis-tance ,breath holding test ,blood oxygen saturation ,arterial partial pressure of oxygen and pulmonary function were tested and compared .The postoperative expectoration cough and pulmonary complication were observed at the same time .[Result]In the experimental group ,breath holding test ,saturation of blood oxygen ,blood pressure and lung function were better than the con-trol group ,and have statistical significance ( P< 0.05 ) . Pulmonary complications for elderly patients with resistance breath training after the rate was significantly lower than the control group ( P<0.05) .[Conclusion] Resistance breath exercise can improve pulmonary function in elderly patients during peri operation period .It can reduce risk and are effective in reducing the occurrence of complications in elderly patients with pulmonary infection after abdominal operation .It is worth promoting in clini-cal practice .%目的：研究抗阻呼气对老年腹部手术患者术前肺功能的改善情况及对患者术后肺部相关并发症发生率的影响。方法：选择2013年1月1日至2013年12月31日我院外科在全麻下行上腹部手术的老年患者（年龄≥65岁）50例，随机分成2组。对照组术前采用常规干预方法，实验组在对照组的基础上加用抗阻呼气法锻炼，比较术前2组患者上楼距离、屏气试验、血氧饱和度、动脉血氧分压以及肺功能等方面的差异，同时观察术后咳嗽咳痰情况及肺部并发症的发生率。结果：实验组在屏气试验、血氧饱和度和动脉血氧分压及肺功能等方面均优于对照组，且有统计学意义（P＜0.05）。行抗阻呼气训练后的老年患者术后肺部并发症发生率明显低于对照组（P＜0.05）。结论：抗阻呼气锻炼能够改善老年患者的肺功能，降低围手术期风险，有效地减少老年腹部手术患者术后肺部感染并发症的发生，在临床上值得推广。
