摘要

The patient centered medical home (PCMH) model for primary health care delivery is associated with increased patient satisfaction with and access to care, as well as greater care coordination for the general population. Comparatively less is understood about the extent to which PCMH tailored to geriatric populations can achieve comparable results. In this symposium, we will present the results of five recent studies conducted within the Veterans Health Administration system which examine the care practices, team composition, and costs associated with implementation and provision of geriatric care and geriatric PMCH (i.e., GeriPACTS). First, Dr. Nicosia’s team will discuss results of an implementation science study to improve functional status measurement— a cornerstone of geriatrics—for older adults in primary care clinics. Next. Dr. Sullivan’s group will share results of a national study of GeriPACT implementation, with a focus on team composition, empanelment processes, and panel characteristics. Dr. Mosely will discuss how her GeriPACT tailored and implemented the VHA Whole Health framework which individualizes psychosocially-grounded health goals and action steps to meet the needs of older adults. Dr. Moye and colleagues will report on findings from recent quantitative research examining integration of mental health care within GeriPACT, and Dr. Intrator will share results from a national data base and propensity techniques to compare GeriPACT costs to those associated with general population primary care teams.
