The clinical efficacy and long-term safety of antihypertensive drugs are optimally tested in large, long-term randomized clinical trials. In 1996, we conducted a meta-analysis of all trials that had assessed major disease endpoints as trial outcomes. The meta-analysis included 18 placebo-controlled trials that enrolled a total of 48 220 subjects. The mean follow-up was about 5 years. All trials employed a step-up regimen with compounds from different drug classes; 2 trials evaluated two regimens. The treatment regimens were classified according to protocol- defined first-line drugs.
