The UK's Air Accident Investigation Branch (AAIB) has confirmed that the engine fan cowls were left improperly latched in an incident on 24 May which involved a British Airways A319 at Heathrow, The aircraft involved lost both doors on take-off, causing a fuel leak and fire in one of the IAEV2500s and forcing the crew to return to Heathrow and land, Maintenance had been carried out on the aircraft overnight.
