There is no doubt that the preliminary accident report into the causes of the March 7, 2007, crash of a Garuda 737-400 at Adi Sucipto Airport in Yogyakarta makes for profoundly disturbing reading. Twenty-one of the 140 people onboard Flight 200 were killed following the decision by the pilot in command, Capt. Marwoto Komar, 45, to carry out a highly unstabilized approach. The crash, which came two months after an Adam Air 737 fell into the sea killing 102, was a major factor in the European Union's move last summer to place the country's airlines on its blacklist of banned carriers en masse. However, the decision by the Indonesian National Transport Safety Committee led by Chairman Tatang Kumiadi to make public the preliminary findings in the Garuda accident represents a significant about face, for it was only a short time ago that accidents in the country received little publicity and official reports rarely surfaced. While the investigation reveals a litany of mistakes and omissions that place the flight crew and Garuda's training program in a poor light, the quality and detail of the report itself are noteworthy.
