The Flight Safety Foundation makes no bones about the biggest threat to the airline industry today—pilots failing to initiate go-arounds when needed. "The problem of go-around policy noncompliance is real and is arguably the greatest threat to flight safety today," the safety advocate states in a March 27 final report on go-around decision-making and execution.
