While the preliminary report on the fatal May 22 accident involving a Pakistan International Airlines (PIA) Airbus A320 leaves many questions unanswered, its contents make clear that two familiar risk areas-unstable approaches and flight deck human factors-featured prominently in the accident sequence. The June 24 Pakistan Aircraft Accident Investigation Board (AAIB) report confirmed that the pilots of PIA Flight 8303 (PK8303) chose to continue their approach to Karachi Jinnah International Airport despite coming in too high and too fast during the initial final-approach phase. Critically, they were also acutely aware of their situation.
