When this column appears, it will be almost exactly three years since the March, 2005 explosion at the BP Texas City, Texas refinery which resulted in 15 fatalities and 180 injuries. Since the incident, three major investigation reports related to this incident have been made available to the public - a BP internal investigation, the BP North American Refineries Independent Safety Panel report (I was a member of this panel), and the United States Chemical Safety and Hazard Investigation Board report. For a major incident such as this, you can often find a lot of information about how the incident occurred, and the lessons from the incident. The reports of the United States Chemical Hazard and Investigation Board (CSB - http://www.csb.gov/) are readily available, and the CSB has also produced videos describing many of the incidents that it has investigated. Many other reports describing less severe incidents are published in journals and presented at various safety conferences.
