The NHS as an organisation is the largest employer and owner of the most extensive and valuable estate in Europe. It also has one of the largest procurement purses in England at ￡20bn per annum (pa). As a waste producer, the English trusts alone generate over 1 m tonnes pa of waste material. The three principal forms of disposal of this waste are incineration (costing ￡33m pa) landfill (￡14m pa) and alternative treatment (eg, autoclaving) for subsequent disposal (f 14m pa) What is not always appreciated is that 75-90 percent of this waste is domestic-type "general" waste, not the clinical fraction associated with healthcare organisations.1 The key constituents of this waste stream are shown in Table 2.
