To the editor: We fully appreciate thecomments on this case of b2-adrenergic bronchodilator–induced thyrotoxic periodic paralysis (TPP).1,2 Weagree that hyperinsulinemia plays animportant role in the pathogenesis ofacute hypokalemia in TPP patients.b2-Agonist is known to stimulateinsulin secretion and may accordingly predispose TPP patients toacute hypokalemia.3 On the otherhand, it should be noted that nearly80% of TPP attacks cannot betriggered by iatrogenic hyperinsulinemia after carbohydrate load.
