Cardiomyoplastyisanexperimentaltreatmentforend-stageheartfailure.Wehypothesizedthattheporcinelatissimusdorsimuscle(LDM)inanexperimentalporcinemodelisasuitablesurrogateforaclinicallyrelevantevaluationofthisconcept.FourteenDanishLandrace(DL)pigsandsixG?ttingenminipigs(GM)werestudied.TheLDMwasevaluatedimmediatelyaftersurgicaldissectionandinvariousphases:phase1(n=4)#8212;outcomeofapartialvascularisolation(vasculardelay),2to3weekspriortoheartwrappinginDLpigs;phase2(n=6)#8212;long-termflapsurvivalofnonstimulatedLDMcardiomyoplastyinDLpigs;phase3(n=6)#8212;outcomeofnonstimulatedcardiomyoplastyinGM;phase4#8212;oneDLpighaddynamiccardiomyoplastyperformedandwassubjectedtolow-intensityLDMstimulationfor8months.brxmlns="http://pub2web.metastore.ingenta.coms/"/brIsolationoftheLDMofDLpigsandGMasapedicledgrafthadnoacutedeleteriousimpactontheglobalbloodsupply.Inphase1a,partialvascularisolationandinsiturecoveryoftheLDMresultedinamuscleencapsulatedinfibrotictissue,whichhamperedalaterheartwrap.Inphase1b,alessextensivedissectiondiminishedfibrosisandallowedsubsequentwrapping.Inphase2,after6weeksofnonstimulatedLDMcardiomyoplasty,theLDMofDLpigswasviable,withexcellentheart-muscleintegration.Inphase3,thesameprocedureappliedinGMyieldedthesameresultasthatinDLpigs,butwithahighersuccessrateowingtothelearningphase.
