Thisstudyexaminestheeffectsofintravenousinfusionofadenosineandsublingualnitroglycerinoncoronaryangiogramsobtainedbycurrent-generationmultidetectorcomputedtomography.Weassessedcoronaryvasodilationatbaselineandafterintravenousadenosine(140#59157;g/kg/min)orsublingualnitroglycerinspray(800#59157;g)in7femaleswine(weight,40.9±1.4kg)byusingelectrocardiogram-gatedcoronaryangiographywitha64-detectorscanner(rotationtime,400ms;120kV;400mA)andintravenouscontrast(300mg/mLiohexol,4.5mL/s,2mL/kg).Cross-sectionalareasofsegmentsintheleftanteriordescending,circumflex,andrightcoronaryarterieswereevaluatedinobliqueorthogonalviews.Imageswereacquiredatanaverageheartrateof73±11beatsperminute.Changesinaorticpressurewerenotsignificantwithnitroglycerinbutdecreased(approximately10%)withadenosine.Ofthe76segmentsanalyzed(baselinerange,2to39mmsupxmlns="http://pub2web.metastore.ingenta.coms/"2/sup),1distalsegmentcouldnotbeassessedafteradenosine.Segmentcross-sectionalareaincreasedby11.3%withnitroglycerinbutdecreasedby9.6%duringadenosineinfusion.Theresultsofthepresentstudyareconsistentwiththepracticeofusingsublingualnitroglycerintoenhancevisualizationofepicardialvesselsandsuggestthatintravenousadenosinemayhindercoronaryarteryvisualization.Thisstudyisthefirstrepeated-measureselectrocardiogram-gatedCTevaluationtousethesameimagingtechnologytoassesschangesincoronarycross-sectionalareabeforeandaftertreatmentwithavasodilator.Thenitroglycerin-associatedchangesinourswinemodelweremodestincomparisonwithpreviouslyreportedhumanstudies.
