Asegmentaltibialdefectmodelinalargeanimalcanprovideabasisfortestingmaterialsandtechniquesforuseinnonunionsandseveretrauma.Thisstudyreportstherationalebehindestablishingsuchamodelanditsdesignandconclusions.Afterethicsapprovalofthestudy,agedewes(olderthan5y;n=12)wereenrolled.A5-cmmiddiaphysealosteoperiostealdefectwasmadeinthelefttibiaandwasstabilizedbyusingan8-mmstainless-steelcross-lockedintramedullarynail.Sheepwereeuthanizedat12wkaftersurgeryandevaluatedbyusingradiography,microCT,andsoft-tissuehistologytechniques.Radiologyconfirmedalackofhardtissuecallusbridgingacrossthedefect.VolumetricanalysisbasedonmicroCTshowedbonegrowthacrossthe16.5cmsup3/supdefectof1.82±0.94cmsup3/sup.Histologicsectionsofthebridgingtissuesrevealedcallusoriginatingfromboththeperiostealandendostealsurfaces,withfibroustissuecompletingthebridginginallinstances.Immunohistochemistrywasusedtoevaluatethequalityofthehealingresponse.Clinical,radiographic,andhistologicunionwasnotachievedby12wk.Thismodelmaybeeffectivefortheinvestigationofsurgicaltechniquesandhealingadjunctsfornonunioncases,whereseveretraumaticinjuryhasledtosignificantboneloss.
