Shigellosisisadiseaseofglobalproportions,withanestimated164.7millionepisodesannuallythroughouttheworldaswellasanestimated1.1millionassociatedmortalitiesindevelopingcountries.Duetoincreasingincidence,andcontinuedemergenceofmulti-drugresistantstrains,Shigellavaccinedevelopmentisconsideredatoppublichealthpriority.Theguineapigkeratoconjunctivitismodel,thebasisfortheSerenytest,remainsthemostreliableinvivoindicatorofvirulenceofShigellastrainsandimmunogenicityandprotectiveefficacyofShigellavaccinecandidates.ThemodeliseffectiveinevaluatingtheabilityofShigellastrainstoinvadethecornealepitheliaofguineapigsandspreadtocontiguouscells,withthemorevirulentstrainscausingulcerativekeratoconjunctivitis.However,analgesiaisnotroutinelyusedtorelievethispainfulconditionbecauseofpotentialimmunomodulationandconfoundingofexperimentalresults.TheobjectiveofthestudyreportedherewastoevaluateuseofbuprenorphinehydrochlorideasananalgesicduringtheSerenytest.Localandsystemicimmuneresponsesweremeasuredinguineapigsgivenbuprenorphineversusthoseresponsesincontrols.Resultsofthisstudysuggestthatbuprenorphine,administeredatananalgesicdoseof0.05mg/kgofbodyweighttwicedaily,canbesuccessfullyusedwiththemodelwithoutsignificantlyaffectingimmunologicevaluationofShigellavaccinecandidates.However,inbuprenorphine-treatedanimals,therewasasignificantincreaseintheamountofmucopurulentoculardischarge,requiringfrequentcleaningoftheaffectedeyes.Additionally,animalstreatedwithbuprenorphinehadsignificantreductioninbodyweight,incomparisonwithsalinecontrols.
