Abstract：Objective To evaluate the implementation of ostomy specialist nurse combined with on-the-spot service arranged through home care mobile platform. Methods 〓The on-the-spot service team was made of ostomy specialist nurses and providers of home care mobile platform. Patients made the telephone appointment or reserved by subscribing the Wechat platform account. The on-the-spot service types, appointment time, home address, contact number of patients were submitted during registration. The ostomy specialist nurses and providers of home care offered the door-to-door service based on the appointment scheduling and mutual consultation. Results 〓The ostomy specialist nurses and home care providers had arranged 150 times of on-the-spot service, without any complaint or adverse nursing events occurred. The implementation of home care mobile platform could improve the awareness of patients on healthcare service, as well as improving the professional achievement of specialist nurses. Conclusion 〓The ostomy specialist nurse combined with on-the-spot service arranged through home care mobile platform is an effective approach of healthcare for housebound elderly population. It is also the value embodiment of specialist nurses.
