To the Editor: In 2004, theCayenne General Hospital and publichealth centers recorded 348 new casesof cutaneous leishmaniasis (CL) inFrench Guiana (1). A case of CL wasconsidered confirmed if cutaneouslesions were present for >2 weeks; thepatient had a compatible epidemio-logic history; and microscopic exami-nation of dermal scrapings, parasitecultivation, or both showed positiveresults for Leishmania. According tothe population estimate given by theFrench National Institute for Statisticsand Economical Studies (INSEE,Cayenne), the incidence of CL in2004 was 0.2%–0.4% and has beenrelatively stable since 1979 (2,3).However, when the annual number ofcases per village were examined, newCL cases were heterogeneously dis-tributed. Saint Elie, a gold-mining vil-lage in the inland neotropical forest,had an apparent incidence rate of25.9% in 2004 and 28.9% in 2005(Figure); risk for infection in this vil-lage was, on average, 65×higher thananywhere else in French Guiana. Wetested samples from 12 random CLpatients with a Leishmania-specificpolymerase chain reaction–restrictionfragment length polymorphism testthat targeted the internal transcribedspacer 1 of ribosomal RNA geneswith primers SSU-12103-D (5′-GGGAATATCCTCAGCACGT-3′)and 5.8S-13333-R (5′-CGACACT-GAGAATATGGCATG-3′) (4).
