In the Rotterdam Aphasia Therapy Studies (RATS), we examine the efficacy of cognitive-linguistictherapy (CLT) directed to the improvement of wordfinding disorders in connected speech.In the first RCT (n= 55) patients were randomized for semantic or phonological therapy(Doesborgh et al., 2004). The hypothesis was that semantic therapy would have a greater effect oneveryday language than phonological treatment (Nickels, 2002). Treatment,1 !4 to 3 hours a weekstarted at 3 to 5 months post onset and lasted until 10 to 12 months post onset. The primary outcomemeasure was the ANELT, scale A (Understandability), a measure of verbal communicative ability(Blomert et al., 1994). After treatment, there was no difference in the ANELT score for both thesemantic and phonological groups. However, at the impairment level, patients improved on asemantic measure after semantic treatment (semantic association) and on phonological measures(repetition nonwords and lexical decision) after phonological treatment. These selective gainssuggest that improved verbal communication was achieved in a different way for each treatmentgroup. Apparently, for patients with both deficits, phonological treatment is potentially as effectivefor improving verbal communication as semantic treatmentA.
