On endoscopy, a 71-year-old man sufferingfrom chronic, severe atrophic gastropathy,with areas of massive completeintestinal metaplasia presented with a35-mm stage IIa/b nongranular, laterallysreading tumor (?Fig. 1). It was narrowband imaging international colorectalendoscopic classification 2 and had atype VI pit pattern and affected 70 % ofthe circumference of the pylorus and extendedthrough the antrum towards thegreater curvature. Biopsies of the lesionshowed a tubular adenoma with lowgradedysplasia, which prompted the decisionto perform DSE.
