The top official of the US Coast Guard (USCG) has acknowledged that decisions made by the captain of El Faro and safety failures by its operator, Tote Services, are among the major reasons that the ro-ro vessel sank, killing all 33 crew members. Weighing in on the deadliest US commercial shipping accident in decades, USCG Commandant Paul Zukunft described the October 2015 event as "tragic and preventable" in his official comments on his agency's investigation into the accident.
