A report has found that a seafarer crushed to death aboard the Bahamas-flagged, 35,998gt open-hatch cargo vessel Toucan Arrow in Portland, Australia, was replacing a broken light on the gantry. The Australian Transport Safety Bureau found that the crane's audible and visual motion warning devices were not fully operational when the fatality occurred in October 2013. A new light was found with a tool bag near the broken one. The assistant electrician was crushed between the aft gantry crane and a cargo hold hatch.
