SIR: Your artide 'Nopoli report criticised as lost opportunity'(8 May, p8) notes my union's concerns that while the MAIB report was incisive, its recommendations fell short of what we would have deemed appropriate to the findings. The Napoli incident was one of those rare occasions when the puMk and the media spotlight falls on the shipping industry, and as a result the investigation into the causes was always going to have immense significance. The ship was managed by a company that has been the focus of previous MAIB reports that highlighted profound concerns about the nature of its operational practices, so that significance was heightened.
