In Germany approximately 2.5 million employees are exposed to natural UV radiation for vocational reasons. Intensive occupational exposure to natural UV radiation over long periods of time is associated with an increased risk of skin cancer. Epidemiological studies have confirmed a positive statistically significant and relevant association of vocation-linked UV exposure and the risk for development of squamous epithelial carcinoma (PEK) and actinic keratosis (AK). Employees in Germany with long-lasting outdoor work have on average an approximately 100 % higher risk for the development of PEK than the rest of the population. Therefore, the prerequisites for a new occupational disease are fulfilled and PEK including multiple AK should be registered as an occupational disease if such an intensive and long-lasting vocational UV radiation exposure has occurred and the clinical criteria are indicative for a vocational causal connection. The current state of medical knowledge is sufficient to acknowledge “squamous cell carcinoma or multiple actinic keratosis of the skin due to natural UV radiation” as a new occupational disease to be included in the annex of the Occupational Disease Act (BKV).
