Developments in the surgical management of hepatic metastases have shown significant improvements concerning inverse and simultaneous procedures, which are oncologically equivalent to the classical sequence. Unfortunately, the question whether patients with resectable metastases profit from neoadjuvant chemotherapy has not yet been satisfactorily answered. The German PANTER study actively recruits patients and should be substantially supported. With laparoscopic liver surgery, even major liver resection can be successfully accomplished with minor blood loss and oncological radicality. This surgical field will be an innovative area and special focus for the coming years. Moreover, recurrent metastases of the liver should undergo repeated resection, even if the tumor occurs a second or third time, as long as the lesions appear to be surgically resectable. Finally, extended surgical resection, such as the ALPPS procedure will be analyzed in more detail by collecting data in an international register to clarify uncertainties of this emerging technique in hepatic surgery.
