A year ago Sunday, a ruptured drainage pipe triggered the nation's third-largest coal ash spill into North Carolina's Dan River, and regulators still aren't sure how much to fine Duke Energy Corp., the company responsible. The leak from the utility's Eden, N.C., coal ash dump took nearly a week to plug -- enough time for gray sludge to coat 70 miles of the Dan River. North Carolina regulators have issued two formal notices for violations of wastewater and stormwater regulations and issued additional notices of violations after finding problems at other coal ash dumps operated by Duke.
