A mid-test failure of the Falcon Hypersonic Technology Vehicle 2 (HTV-2) in August 2011 was probably due to "unexpected aeroshell degradation, creating multiple upsets of increasing severity that ultimately activated the [vehicle's] flight safety system", a US Defense Advanced Research Projects Agency investigation has concluded. The test came 16 months after the first test, which also ended in failure after telemetry signals were lost about nine minutes into the flight.
