The procurement of major capital equipment by Australia's Department of Defence (DoD) suffers from a flawed management structure in which no-one owns risk and which is seemingly incapable of learning from its mistakes, according to a parliamentary report released on 31 August. The report, by the all-party Senate Foreign, Defence and Trade References Committee, said the current management structure in the DoD had produced an organisation "that lacks a robust risk-management regime; an organisation where its personnel are insensitive or unresponsive to risk, where no-one owns risk".
