An independent review into the loss of a UK Royal Air Force (RAF) BAE Systems Nimrod MR.2 surveillance aircraft over Afghanistan has found that it was caused in large part by a cocktail of "incompetence, complacency and cynicism" within the Ministry of Defence (MoD) and its industry partners.rn'The Nimrod Review', released on 28 October, found that the MoD and RAF allowed safety to suffer in pursuit of financial savings and that the incident was entirely avoidable. This was compounded by the fact that a Nimrod Safety Case, established by the MoD and industry partners in 2002 to prevent such accidents, had done "a lamentable job from start to finish".
