Reliably communicating critical test results remains problematic; 27% of the 1,405 hospitals surveyed in 2009 by The Joint Commission did not comply with NPSG.02.03.01. The goal, first required in 2005 and the subject of a Journal issue and subsequent book, remains a frequent subject of discussion and controversy on the National Patient Safety Foundation listserve. Clearly, health care organizations are still struggling to address the complexities of test-result management.rnIn the current issue, Singh and Vij distill the lessons from the literature and their experience in the Department of Veterans Affairs (VA) system to offer recommendations for institutional policies on communication of abnormal diagnostic test results. This contribution of a high-level look at their VA hospital policy-the evolution, the thinking, and lessons learned in its development and implementation-is valuable.
