Robert Alford’s illuminating analysis of the organisationnof health care . rst appeared almost 30 years ago at antime when many health care systems around the world,nincluding that in the USA which is the subject ofnAlford’s study, were allegedly in a state of crisis innrespect of rising demand and costs. The health carenreform industry was in its infancy. Little appears to havenchanged over the intervening years. A sense of crisisncontinues to pervade health care systems regardless ofntheir funding or structure. As a consequence, they arenbeing subjected to continuous reform aimed at addressingndemand- and/or supply-side pressures.
