Patient and public involvement is central to the Britishngovernment’s modernisation of the National HealthnService. A ‘patient-centred service’, it is argued, isnnecessary to improve health outcomes, provide anbetter experience of care for patients and reconnectnthe health service with the communities it serves.1 Newnstructures are being put in place to enable and promotenthese changes, including patient and public involvementnforums in every NHS Trust, and public membership andngovernors of foundation hospitals, backed by a legalnduty to consult the public on all significant changes innservices or provision.2 Parallel approaches are developingnin Canada, Australia, the USA and in some othernEuropean countries.nThe paper from Julia Abelson and colleagues (ppn205–212) from Canada is a timely reminder of thendifficulties of, and opportunities for, public involvement.nThey find, significantly, that citizens are bothnconscious and critical of the processes of consultationnand involvement, and ‘discerning about their publicnparticipation experiences’. Citizen participants are tonsome extent cynical about the intentions and purposesnof public consultations, but are, nonetheless, ‘optimisticnand determined to contribute’.
