Accuracy of Recalled Experience: Depression Measurement's Enduring Illusion

Depression may be the most frequently studied phenomenon in clinical psychology, psychiatry, and psychiatric epidemiology. Santor, Gregus, and Welch's (2006; this issue) meticulous empirical review reveals that although there are now a plethora of depression severity scales, their item content has not changed over several decades, and what we know about the effectiveness of depression treatment turns in large part on the quality of two scales, the Beck Depression Inventory and the Hamilton Rating Scale for Depression. These measures, along with less frequently used depression scales and virtually all symptom severity scales, require respondents to recall their symptoms and emotional experience over a period from several days to 2 weeks. As investigators and clinicians, we have assumed that the relative ease with which research participants and patients complete these scales reflects the accuracy of their recollections. Yet in the years since these depression measures were developed, numerous investigations have demonstrated that such symptom reports are fraught with recall decay and systematic bias. No matter how carefully the content of new depression scales is refined, these measures will be deficient unless we come to grips with the limits of recalled experience. This limiting factor in depression measurement is the focus of my commentary.
