Disease management plays a vital role in optimizing the health of the public and helps to moderate the costs of care for the growing number of Americans treated for chronic conditions. More than 105 million Americans suffer from at least one chronic condition. Asthma, diabetes, and congestive heart failure are among the leading chronic diseases affecting Americans and account for nearly $400 billion in health care expenditures each year, according to the Centers for Disease Control and Prevention's National Center for Chronic Disease Prevention.
