Not surprisingly, fall protection was the most-cited safety violation in 2016, says OSHA's preliminary list. The agency's 2016 top 10 violations are: 1) fall protection (6,929 violations); 2) hazard communication (5,677); 3) scaffolds (3,906); 4) respiratory protection (3,585); 5) lockout/tagout (3,414); 6) powered industrial trucks (2,860); 7) ladders (2,639); 8) machine guarding (2,451); 9) electrical wiring (1,940); 10) electrical, general requirements (1,704). "One remarkable thing about the list is that it rarely changes," OSHA says. "Year after year, our inspectors see thousands of the same on-the-job hazards, any one of which could result in a fatality or severe injury." According to DOL, more than 4,500 workers are killed on the job each year and approximately 3 million workers are injured.
