The space shuttle Challenger exploded above the Florida coast shortly after its launch on Jan. 28,1986, and its sister shuttle Columbia disintegrated in the skies above Texas during reentry on Feb. 1, 2003. During the Columbia investigation, astronaut and investigation board member Sally Ride delivered one of the more memorable quotes: "I think I'm hearing an echo here," because while the specific failures in technology for each shuttle were different, both disasters shared critical organizational factors.
