On Nov. 4 and 5, 2010, ASSE sponsored a symposium titled Rethink Safety: A New View of Human Error and Workplace Safety. It was successful with 350 people in attendance. I spoke on "Designing Out the Error-Provocative." Since I was scheduled to speak some time after lunch on the last afternoon, I did not expect a large attendance. I was pleasantly surprised to see the contrary. There was more interest than I had anticipated.
